Appeals Process
If you have a complaint relating to a determination of benefits, please contact Delta Dental of Wisconsin directly using the ETF-specific customer service line: 844-337-8383, or via email at etfcustomerservice@deltadentalwi.com, and we will assist you in trying to resolve the matter.

If the matter is not resolved, you may choose to submit a formal grievance to Delta Dental of Wisconsin. Formal grievances must be submitted within ninety (90) days of the original denial, and may include additional documents, written comments or other information relevant to the determination.
